At your hearing, the Tribunal must consider your views and wishes.

This form will help you plan what you want to tell the Tribunal at your hearing.  You may bring it with you to the hearing.

Please download the form and submit via the MHT Document Lodgement Portal.

If you do not want to attend the hearing, you can complete this form and submit via the portal to the Tribunal at least one day before your hearing.

If you are in hospital, you may ask the treating team to send the form to the Tribunal for you.
