Tell the Tribunal your views and wishes

At your hearing, the Tribunal must consider your views and wishes.

This form will help you plan what you want to tell the Tribunal at your hearing.  You may bring it with you to the hearing.

If you do not want to attend the hearing, you can complete this form and email it to the Tribunal at least one day before your hearing.

The easiest way to do this is to take a picture of both sides of the form and email it to the Tribunal at registry@mht.wa.gov.au.

If you are in hospital, you may ask the treating team to send the form to the Tribunal for you.
