Notice of Representation Forms

Lawyers acting for clients in the Mental Health Tribunal must complete a Notice of Legal Representation form to inform the Tribunal when they begin or cease acting.

Advocates should tell the Tribunal whether they are instructed to attend a hearing by lodging a Notice of Advocate Representation form.
